Senior Connections
Our comprehensive supportive services program for seniors, addresses our residents' and community members' physical and mental health, social, nutritional and spiritual needs. It educates and empowers members of the senior community and provides them with the support and resources they need to remain in their homes, living as independently as possible for as long as possible.
Program Hours are 9:00 am to 5:00 pm. or in home. Bi-lingual assistance is provided.
Health Services
- Five-day on site nursing services Services
- On-site health and blood pressure screenings
- Health and wellness workshops, from general topics such as "Seniors and Safety" to specifics about diabetes, hypertension and arthritis
- 24 hour primary care access
- Physician referrals
- Vaccinations (flu)
- MyMEDS medication information and management and legal services assistance, in conjunction with local pharmacies and Roosevelt Hospital Emergency Room staff
- Doctor home visits by geriatrician team
- Home health aides
